Capotril® is a selective inhibitor of angiotensin converting
enzyme (ACE) and was the first ACE inhibitor to be used
clinically. ACE is a peptidyl carboxypeptidase that catalyzes
the conversion of angiotensin I to the active octapeptide
angiotensin II in blood and other tissues. Angiotensin II
increases blood pressure through its direct vaso-constrictor
effect and, indirectly, through stimulation of aldosterone
secretion and the sympathetic nervous system. ACE inhibition
results in reduced plasma angiotensin II and as a result of the
loss of feedback inhibition, plasma renin activity increases.
